Plantar heel pain is a common and debilitating condition.  South West Podiatry are committed to helping more people and dedicating our time to education and promoting foot health.  In this series of blogs, we discuss one of the most common form of foot pain for the general population and over the next few weeks we will discuss the signs and symptoms of plantar fasciitis/plantar heel pain.  It has been reported that up to 20 % of the population within the UK will suffer from symptoms of plantar fasciitis.  This is generally of varying degrees and plantar fasciitis can be one of the most debilitating conditions of the foot and ankle.

Part 6 :

In part 4 we discussed the excellent work by Rathleff et al and showed you how to complete a simple plantar fascia loading programme under guidance from your podiatrist.  Plantar fascia pain is common and when you start to read about plantar fasciitis, you start to appreciate the complexity of the foot and ankle.  One of the common comments we hear in clinic is ” I didn’t appreciate my feet until they started to hurt “.  Well, with over 26 bones, 33 joints, 100 + muscles, tendons and ligaments, not to mention a large network of blood vessels, nerves and skin.  You start to appreciate that the foot is more complex than two blocks of wood at the end of your body.

There is a growing body of research in the field of building intrinsic muscle strength of the feet to help a wide range of foot and ankle conditions, including plantar fasciitis.  With around 19 true intrinsic foot muscles and 4 layers of intrinsic muscles of the feet, its no surprise that when we train these muscles, symptoms of pain reduce.  South West Podiatry has excellent results and outcomes for a wide range of conditions when we work with you to train and condition these smaller, intrinsic muscles of the feet.  There are many ways of conditioning these muscles, but one of our favourite methods is an exercise called ” toe curls “.  The aim is to engage the muscles on the bottom of the feet, spreading the toes and improving strength.  Check the video out below.

When would you strap the plantar fascia?

Within the last few blogs, we have discussed stretching the calf complex, loading the calf and plantar fascia and self / assisted massage.  These exercises should guided by your podiatrist as some of the exercises will be more suitable than others which would depend on the severity of your plantar fascia symptoms.

As a team of foot specialists, we have many patients referred into our service from allied health professionals, like chiropractors, osteopaths and physiotherapists.   One of the clinical tests which is very common practice within these allied health professions, is to strap the arch of the foot to decrease stress within the plantar fascia.  The strapping is usually applied with sports tape/low dye tape and can remain on the foot for between 3-5 days.  Strapping the foot is a great way to find out if this can temporarily reduce symptoms of arch or heel pain.  If this test is positive and it helps to reduce plantar fasciitis/ plantar heel pain, then specialist insoles or medical orthotics are likely to be of significant benefit in reducing plantar fascia tensile stress.  The aim of an orthotic device is to decrease plantar fascia loading during ambulation, running or sports.  Find out more about orthotics here:  Click here

In parts 1 and 2 we discussed how to stretch the gastrocnemius and soleus muscles.  Within part 3 we will also show you how to stretch the plantar fascia.  Stretching, massaging and moving the fascia layers can significantly help to reduce arch and heel pain caused by plantar fasciitis.  Self or assisted massage can release tension within the tissues under the arch, help to break down muscle knots and provides some short term pain relief.  Foot mobilisation therapy ( FMT ) and Tool assisted massage works in a similar way by releasing stiff joints of the foot and ankle whilst the tool assisted massage breaks down tissues to promote healing.  It is also thought to decrease sensitivity in the arch by influencing C nerve fibres which are responsible for causing hypersensitivity and increased pain in a specific location of the body.

Part 4 :

Parts 1 and 2 looked at flexibility of the calf complex.  In particular the gastrocnemius and soleus muscles.  Part 3 discussed how releasing the myofascial layers and stretching the plantar fascial layers can also have a positive outcome and start to decrease symptoms of heel and arch pain.  Part 4 looks at some of the research by Rathleff et al, looking at how loading the plantar fascia and improving strength within the calf complex, feet and ankles can significantly improve pain levels at 12 months. Part 2 :

calf muscles

This weeks part 1 rehabilitation tool looked at a simple calf stretch for a large calf muscle.  In part 1 of our free exercises which can help improve calf flexibility and decrease symptoms of heel pain/arch pain, we now look at the other muscle of the calf.  We have two main muscles within the calf, one called the gastrocnemius and the other called the soleus.  Both these muscles form part of the Achilles tendon and insert into the back of the heel.  As you already know from part 1, 50 % of patients have tight calf’s associated with plantar fasciitis so it would help to stretch the second largest muscle within the calf complex under guidance from your podiatrist.  The soleus is a smaller muscle but plays a large role in calf flexibility.

Plantar fasciitis – Stretches

Causes

The most common cause of plantar heel pain ( fasciitis/fasciopathy ) relates to faulty structure of the foot and overuse. For example, people who have problems with their arches, either flexible overly flat feet or high-arched feet, are more prone to developing plantar heel pain. 
Wearing non-supportive footwear on hard, flat surfaces puts abnormal strain on the plantar fascia and can also lead to plantar fasciitis. This is particularly evident when one’s job requires long hours on the feet. Obesity may also contribute to plantar fasciitis. It is also a very common running and sporting injury.

Symptoms

  • Pain on the bottom of the heel.
  • Pain in the arch of the foot.
  • Pain the is worse in the morning.
  • Pain that increases over a period of time.

Symptoms are often described as painful when you get up in the morning or after sitting for long periods of time. After a few minutes of walking the pain decreases, because walking stretches the fascia. For some people the pain subsides but returns after spending long periods of time on their feet.

Treatment :

Evidence points towards a tight / short calf complex being responsible for the cause of plantar fasciitis in 50 % of cases.  Here is an example of an exercise which your podiatrist may give you to manage your condition:

Foot Pain | Morton’s Neuroma | Maidenhead

What is a Morton’s Neuroma?

Foot pain in the ball of the foot, or shooting pain in the toes. One of the most common diagnosis of these symptoms is a neuroma https://www.swpodiatry.co.uk/common-conditions/neuroma/.

Morton’s Neuroma has several similar names; Mortons Metatarsalgia, Morton’s Neuralgia, Plantar Neuroma and intermetatrsal Neuroma.

It is a benign bundle of nerve, most commonly of the third and fourth toe webspaces but can occur in any of the webspaces of the forefoot. Typically this is characterised by numbness and shooting pain into the toes and burning sensations on the ball of your feet. The pain is often relieved by removing footwear, massaging or rest. Symptoms are exaggerated by walking and exaggerated by exercise. Other diagnosis of shooting pains in the toes include; Diabetes, Anaemia and Radiculopathy, so it is very important to seek advice from a foot specialist if you develop any of these symptoms.

Signs and Symptoms

  • Paraesthesia.
  • Burning sensation.
  • Shooting pains.
  • Pain on weight bearing.
  • Numbness.

Diagnoses:

  • Direct pressure applied between the metatarsal heads should replicate the symptoms.
  • Forefoot squeeze test known as a mulder’s click.
  • An ultrasound examination or X rays may be requested to rule out differential diagnosis such as stress fractures.

How can our team help you if have a Neuroma?

South West Podiatry at Maidenhead will assess your foot and provide you with a clinical diagnosis. An Ultra sound examination or MRI may be requested. Once your Podiatrist has made a clinical diagnosis, you will be provided with a treatment plan which may include:

Treatment:

  • Gait analysis / Biomechanical Examination
  • Orthotics to open out metatarsals pressing the nerve and reduce the mechanical load.
  • Footwear advice.
  • Padding or Strapping.
  • One or more Corticosteroid injections.
  • If conservative treatments fail, patients may be referred for surgical intervention.
